About AI Endolaser
AI Endolaser is a minimally invasive laser treatment for jowls, a softening jawline, fullness under the chin and early loosening of the neck. Through a few tiny entry points, a laser fibre finer than a hair is placed just beneath the skin, where its heat tightens the tissue that supports the skin, stimulates new collagen and can reduce small pockets of fat. It is also used for the lower face after weight loss, where skin has not tightened back on its own.
AI Endolaser at Aarva is performed by Dr Shivani Malhotra, a practising GP who leads our skin and regenerative medicine, under local anaesthetic in our practice on Midland Drive, Royal Sutton Coldfield. We are honest about where it stops. AI Endolaser is most suited to mild to moderate looseness and small pockets of fat. It is not a facelift. Where there is a lot of loose skin, or the jawline has changed because of bone or volume loss, it will not do enough on its own, and we will tell you so.

A fine laser fibre is placed in the layer just beneath the skin. Its heat makes the fibrous tissue that supports the skin contract, which is why some tightening can show early. Over the next three to six months the skin lays down new collagen and firms further. Where there is a small pocket of fat, the heat can break some of it down, and the body clears it naturally.
Both describe the same kind of treatment: a laser fibre placed beneath the skin to tighten it and reduce small pockets of fat. Endolift is one brand name. At Aarva we use the AI Endolaser system.

It can reduce a small pocket of fat and tighten the skin under the chin. Not every double chin is fat, though. Sometimes it is loose skin or the shape of the jaw and chin, which is why a doctor examines you first.
It suits mild to moderate looseness and small pockets of fat in the lower face. It is not suitable during pregnancy or breastfeeding, with an active infection in the area, or with a bleeding disorder. If you take medicines that thin the blood, have had fillers or threads in the area recently, or have a condition that affects healing, we discuss it at consultation.
Swelling, bruising, tenderness and firmness under the skin are expected and settle over days to weeks. Less commonly, there can be temporary numbness, uneven contour while the tissue settles, small marks at the entry points or infection. Rarely, there can be a burn, lasting unevenness, scarring, or irritation of a nerve along the jawline causing temporary weakness of the lower lip, which usually recovers over weeks to months.
The local anaesthetic stings briefly. After that, most people feel pressure and warmth rather than pain. The area can be tender for a few days.
Most people take two to five days away from work or social plans. Swelling settles over one to two weeks, and firmness under the skin over several weeks. Avoid strenuous exercise, saunas and hot baths for about a week.
Some tightening can show once the swelling has gone down. The fuller result develops over three to six months as new collagen forms.
The tightening and fat reduction are long-lasting, but your face keeps ageing. Many people maintain the result with radiofrequency, and some choose a second Endolaser session later on.
For early loosening, it can be. It does not lift and remove skin the way surgery does. If surgery would serve you better, we will tell you.
